200406: COVID Responder Form for Andrew's College - staff mentors/senior staff
You should complete this form if you are assisting any staff member or residential student who;
1. has any cold or flu like symptoms
2. is concerned they have been a contact of a person with coronavirus Covid19
3. has been diagnosed with coronavirus Covid19

This form is only for the internal use by and for St Andrew's College Members and will only be viewed by the St Andrew's College Principal, Vice Principal and Director of Operations and staff mentors.

5. Are they ACUTELY UNWELL (i.e. So breathless they are unable to speak in sentences, Unconscious or drowsy, Skin turning blue or pale, Pale with unusually cold clammy skin associated with rapid shallow breathing) Do they need to go to hospital? *
6. Have they been diagnosed by a doctor with Covid19? *
7. Have they been in close contact in the past 14 days with a person who is a confirmed case of coronavirus (COVID-19) including 24 hours before they became unwell? OR Are they a household contact with someone who has been diagnosed with or is a suspected case of coronavirus (COVID-19)? *
Call triple zero (000) right now and ask for an ambulance, tell the call taker that the person you’re helping could be a suspected case of coronavirus (COVID-19).
If you’re using a mobile phone and triple zero (000) isn’t answering, try calling 112.
Tell the paramedics when they arrive that the person you’re assisting could be a suspected case of coronavirus (COVID-19).
Tell the person:
'You are a coronavirus Covid19 case.
You are at high risk of transferring the infection to others.
DO NOT have any physical contact with anyone.
If you are a staff member, follow the advice of your doctor and do not leave your home, do not attend work.
If you are a student in residence at College, do not leave your room and please wait for further instructions. I will get back to you as soon as I can. College will organise your care and meals and a transfer to a en suite room.
Please stay in your room and wait for further instructions.

Tell the person:
'You are at risk of being infected with coronavirus Covid19.
Please stay in your room, let your housemates know and do not have any physical contact with anyone.
If you are a staff member, contact your GP for further advice and do not attend work.
DO NOT have any physical contact with anyone.
If you are a student in residence at College, do not leave your room and please wait for further instructions. I will get back to you as soon as I can. College will organise your care and meals and a transfer to a en suite room.
Please stay in your room and wait for further instructions.

Tell the person, even though you have not had any risks, you have a flu like illness so you need to put on your mask and attend the Covid Clinic at RPAH to get tested.
The NSW government has updated testing protocols for our local area and while we want to exclude any infection in our community that could be Covid19.
Under the guidelines you are a suspect Covid19 case until your results come back.
This generally takes about 1-2 days.
I will organise to transfer to an ensuite room, if you do not already have one, until you get your tests results back. Please quarantine yourself in that room until your results come back.
This means:
- no contact at all with anyone.
- do not leave your room for a walk.
- Do not leave your room to get food.
- Communicate with your flat mates and or staff mentor via text, app or phone call to organise food.
After you receive your result, let me know and do not leave quarantine until I let you know you are allowed to do so.

If you are a staff member, follow the advice of your doctor, rest at home and contact your manager.

You are not currently high risk for coronavirus Covid19. If you have any other health concerns, contact your doctor.
